The NDIS Commission's review of the Practice Standards (a proposed quality framework and changes to the standards and how they are assessed) is still being considered; nothing from it is in force.
If the family wants a key worker, the provider works with the family and other providers to find a suitable key worker to coordinate information, services and supports for the child.
- Record of the family's decision on whether to have a key worker
- Key worker allocation records naming the agreed key worker
- Notes of discussions with other providers on key worker choice
- Key worker allocated without asking the family
- Families not told the key worker option exists
The provider establishes close collaborative links with the family and other collaborating providers so the team around each child is coordinated, works toward shared goals and avoids duplicated or conflicting advice.
- Team meeting minutes involving the family and other providers
- A shared contact list for the team around the child
- Joint goal or plan documents agreed across providers
- Each provider working to separate, conflicting goals
- No record of contact with other providers
With the family's consent, the provider communicates and shares information, knowledge and skills between the family, itself and other collaborating providers, so everyone working with the child has what they need.
- Signed family consent to share information, naming the parties
- Correspondence or reports shared with collaborating providers
- Records of joint sessions or skill-sharing between providers
- Information shared without documented consent
- Consent forms that do not name who may receive information
Where relevant, the provider collaborates with other supports and services so that transition and exit planning, for example to school or another provider, meets the needs of each child and family.
- Transition plans for starting school or leaving early childhood supports
- Handover reports to schools or receiving providers, with consent
- Transition meeting notes including the family
- Exits with no transition plan or handover
- Receiving services not contacted before exit
