Because the community trusts practitioners to sign only statements they know or reasonably believe are true, good practice means the practitioner: (a) is honest and not misleading in reports and certificates and signs only documents believed accurate; (b) takes reasonable steps to verify content before signing and does not deliberately leave out relevant information; (c) prepares or signs agreed documents and reports within a reasonable, justifiable time; and (d) states the limits of their knowledge and does not give opinion beyond them when giving evidence, in person or in writing.
- Copies of certificates issued, linked to the consultation note supporting them
- Report files showing the sources verified before signing
- Log of report requests with dates received and issued
- Certificates issued without a consultation or backdated
- Expert opinions outside the practitioner's field
