Practitioners have responsibilities and rights in legitimate investigations of their own or a colleague's practice, and are advised to seek legal or insurer advice. Good practice means the practitioner: (a) cooperates with any legitimate inquiry into a patient's treatment and with the complaints processes or investigations covering their work, giving accurate, honest information when asked; (b) discloses information relevant to an investigation into the conduct, performance or health of themselves or a colleague to anyone entitled to ask; (c) cooperates with legitimate investigations of suspected National Law offences; and (d) assists the coroner at an inquest or inquiry into a patient's death by answering enquiries and offering all relevant information.
- Correspondence with Ahpra, the Board, a complaints entity or the coroner showing timely, complete responses
- Record of legal or indemnity insurer advice obtained
- Failure to respond to an investigator's request by the due date
- Records altered after an investigation began
