A practitioner applies to renew no later than one month after the registration period ends (registration continues during that month and while a timely application is decided), in the approved form with the fee (a late fee after expiry) and an annual statement. The statement declares that the applicant has no impairment, has met any recency of practice requirement in an approved registration standard, has completed the continuing professional development required during the previous period, did not practise without appropriate professional indemnity insurance and will not practise without it; and gives details of any change in criminal history, any withdrawal or restriction of hospital or facility practice rights for conduct, performance or health, any Medicare disqualification (where the Health Insurance Act 1973 allows disclosure), any complaint to a registration authority or similar body, and anything else a registration standard requires. Non-practising renewals omit the recency, CPD, past-insurance, practice-rights and Medicare items.
- Renewal confirmation for each period with the date lodged against the registration expiry date
- Copy of the annual statement with each declaration answered
- Recency of practice record (hours or periods of practice) supporting the declaration
- Employer register re-check log after each annual renewal date
- Renewal lodged more than a month after expiry, so registration lapsed and practice continued unregistered
- Recency of practice declared without a record of hours after a career break
- A complaint made to a health complaints entity not disclosed because it was later dismissed
