The system must set procedures for identifying, managing and resolving incidents that state: how incidents are identified, recorded and reported; to whom they are reported; the person responsible for notifying reportable incidents to the Commissioner; how the provider supports and assists affected persons with disability to protect their health, safety and wellbeing, including information about advocates such as independent advocates; how affected persons are involved in managing and resolving the incident; when the provider investigates to establish causes, effects and contributing operational issues, and the nature of that investigation; and when corrective action is needed and what it is. Procedures may be scaled to the seriousness of the incident (s 10(2)).
- Incident procedure naming the reporting line and the nominated person who notifies the Commissioner
- Participant file entries showing support offered after an incident, including advocacy information
- Investigation trigger criteria and a completed investigation report
- Corrective action log linked to incident numbers
- No named person accountable for Commission notifications
- Affected participants not told about access to an independent advocate
