Requirement, our statement of the clause

The system must collect statistical and other information about incidents so the provider can review the issues incidents raise, identify and address systemic issues, and report information to the Commissioner when asked.

  • Periodic incident trend report by type, site and participant
  • Board or management minutes discussing incident trends and resulting actions
  • Record of any incident data supplied to the Commission on request
